BACHELOR OF SCIENCE IN NURSING
Admission Criteria
1. GWA of at least 85%
2. CFAT score of 94
3. Belongs to the upper 40% of the graduating class in high school (exemption for MMSU High School System)
4. Of good moral character
5. Physically fit/with pleasing personality
Admission Requirements for Qualified Applicants (must be original copy)
1. CFAT result
2. High school report card (Form 138)
3. Certificate of good moral character from the principal and or guidance counselor
4. Authenticated birth certificate from the National Statistics Office (NSO)
5. 4 pieces 2x2 ID picture
6. Filled up bio-data sheet
7. Long sized brown envelop
Applicants are wait-listed if
1. They obtained a CFAT score of 90-93 and have a GWA of at least 85% and meet all other requirements for admission
2. They obtained a CFAT score of 94 and above with a GWA of at least 83% and meet all other requirements for admission
3. They graduated as valedictorian or salutatorian who qualify based on admission criteria but did not prioritize nursing

Admission Criteria
1. CFAT score of at least 70
2. General weighted average (GWA) of at least 85
3. Grade in Science and Mathematics of at least 80
4. Physically and mentally fit
5. Undergo panel interview
Admission Requirements
1. CFAT result form
2. High school report card (form 138)
3. Certificate of good moral character
4. Original NSO birth certificate
5. National College Assessment Exam (NCAE) score sheet
6. 3 pieces 2x2 recent, colored ID picture
7. Long-sized brown envelop
Applicants are wait-listed if
1. They indicated BS Pharmacy as the first priority but did not qualify in CFAT cut-off: 65-69, provided they meet all other requirements for admission
2. They indicated BS Pharmacy as the second priority: CFAT score of 70 or better, provided they meet all other requirements for admission
3. BS Pharmacy is neither 1st or 2nd priority: CFAT score of 75 and or better, provided they meet all other requirements for admission

BACHELOR OF SCIENCE IN PHYSICAL THERAPY
Admission Criteria
1. CFAT rating score of not lower than 60
2. High school GWA of at least 80% or higher
3. Physically fit and have passed the medical and dental examination; certification to be issued by the University Physician
4. Must submit the following requirements:
4.1 Original form 138 (high school report card)
4.2 Certificate of good moral character from school principal/appropriate school authority
4.3 One photocopy of high school diploma
4.4 One original NSO birth certificate on security paper
4.5 Two pieces, colored 2x2 picture with white background
4.6 One accomplished bio-data form
4.7 One long brown envelop
4.8 NCAE score
